What Is Acute Phosphate Nephropathy (APN)?
Acute phosphate nephropathy (APN) is a rare but very serious type of kidney problem that has occurred in patients who used an oral sodium phosphate solution (OSPS) as a bowel cleanser prior to undergoing a colonoscopy or colon-rectal surgery.
Acute phosphate nephropathy is also sometimes called acute nephrocalcinosis. Acute phosphate nephropathy can lead to acute renal failure, which can require medical monitoring, emergency and ongoing dialysis, kidney transplant, and sometimes even result in death.
What Is the Connection Between OSPS and Acute Phosphate Nephropathy?
Bowel cleansing with OSPS products such as Fleet Phospho-soda, OsmoPrep or Visicol can cause dehydration, decreased intravascular volume, and hyperphosphatemia (an abnormally high level of phosphates in the blood).
Hyperphosphatemia increases the levels of phosphates in the kidneys. The decreased intravascular volume leads to further increased phosphate concentration in the kidneys. This can in turn lead to creation of calcium-phosphate crystals in the kidneys, which leads to acute phosphate nephropathy.
According to the Food and Drug Administration (FDA), there are at least 21 proven cases of acute phosphate nephropathy in patients who used an OSPS as a bowel cleanser prior to colonoscopy or colon-rectal surgery.
OSPS products include Fleet Phospho-soda, OsmoPrep tablets, Visicol tablets, Wal-Phosphate products, certain CVS brand bowel cleansing products, and other similar products.
Do Not Avoid Colonoscopy
Colonoscopy is a highly effective medical diagnostic tool for the early detection of several different types of cancer.
If you are scheduled for a colonoscopy but concerned about the risk of acute phosphate nephropathy or chronic kidney disease, talk to your doctor about your concerns. Your doctor can perform a screening procedure before the colonoscopy to help determine what type of bowel-cleansing product will be safe for you to use.
