Risk Factors for Kidney Failure

After Using an Oral Sodium Phosphate Product Before a Colonoscopy

In recent years medical studies are finding a link between acute kidney failure, chronic kidney disease and the use of oral sodium phosphate products like Fleet Phospho-soda OsmoPrep and Visicol.

Doctors often recommend the use of an oral sodium phosphate product as a bowel-cleansing regimen prior to a colonoscopy or colon-rectal surgery. However, the Food and Drug Administration (FDA) now recommends that certain types of individuals not use these products as bowel-cleansers prior to a colonoscopy - because of the risk of acute kidney failure.

Risk Factors for Kidney Failure

According to the FDA, physicians should avoid the use of an Fleet Phospho-soda, OsmoPrep, Visicol and other oral sodium phosphate products in certain types of patients to decrease the risk of acute kidney failure. The FDA recommends caution using these products in any patient over 55 and also makes the following suggestions:

  • Avoid using these products in patients with kidney disease, impaired renal function or perfusion, dehydration or uncorrected electrolyte abnormalities.
  • Avoid concomitant use of laxatives containing sodium phosphate.
  • Use these products with caution in patients taking diuretics, angiotensin-converting-enzyme (ACE) inhibitors, angiotensin-receptor blockers (ARBs), and non-steroidal anti-inflammatory drug (NSAIDs).
  • Encourage patients to take the correct dose and drink sufficient quantities of fluids during bowel cleansing.

Other risk factors may include:

  • A history of heart disease, congestive heart failure, cirrhosis, or nephrotic syndrome
  • Acute or chronic kidney disease
  • Bowel disease

If you are scheduled for a colonoscopy but concerned about the risk of kidney failure, you should not avoid the colonoscopy. Talk to your doctor about your concerns. Your doctor can perform a screening procedure before the colonoscopy to help determine what type of bowel-cleansing product will be safe for you to use.
